Essentially, an Individual Service Plan (ISP) is a written personal life plan. It is designed to help individuals with developmental disabilities attain their individualized valued outcomes. The ISP encapsulates your loved ones’ needs and wants as well as their unique network of supports and services. This network is known as their circle of support.

Individualized Service Planning concentrates on planning all activities on your loved ones’ needs and desires. These activities unite the most important people in their lives—you and the other people who matter to them—to help them reach their goals. Often, Individualized Service Planning is referred to as Person-Centered Planning. Our service coordinator will help your loved ones with the process of achieving their valued outcomes as well as the supports and services they need to have happy, successful lives.
